Proof of Coverage
Follow these steps if you need a certificate of insurance as proof of coverage when applying for a work visa with your local embassy.
- Call Chubb Representative Diane Basa at 302-476-6194 or send an email to diane.basa@chubb.com.
- Chubb will complete a Travel Certification form within 1 – 2 business days. Submit the completed form to your local embassy and carry it with you during your trip. See a sample Travel Certification here.
- If your request is urgent, indicate this in your email to Chubb.
